Family Member Dental Plan

ucciLogo.jpgThe Tricare Family Member Dental Plan is underwritten by United Concordia Companies, Inc. (UCCI). Enrolled members should receive a booklet from UCCI describing the enhanced coverage as well as a Tricare Dental Plan (TDP) identification card for every enrolled member.

The Tricare Family Member Dental Plan is underwritten by United Concordia Companies, Inc. (UCCI). Enrolled members should receive a booklet from UCCI describing the enhanced coverage as well as a Tricare Dental Plan (TDP) identification card for every enrolled member.

UCCI now handles the enrollment process exclusively. Eligible individuals interested in becoming enrolled may call UCCI at 1-888-622-2256 or may access the UCCI web site at www.ucci.com and download an enrollment form. An online enrollment feature will be added soon. Local personnel offices, dental facilities or health benefits advisors will also have enrollment forms.

All new enrollees must submit payment equal to the member's portion of one month's premium. Enrollment becomes effective the 1st day of the month following the month in which the sponsor enrolls. For example: if the sponsor enrolls on 6 February, the family will be eligible for benefits on 1 March.

Some of the improvements to the enhanced TDP include eligibility, benefit maximums, cost shares and new procedures are listed below (see the new booklet for complete information):

Eligibility: Sponsor only need to have 12 months remaining on Active Duty. Selected Reservists and IRR Members and their family members may now enroll under a separate single or family plan.

Maximum Benefits: The annual maximum yearly benefit will increase to $1200 per benefit year (01 Feb - 31 Jan) per member. Certain diagnostic procedures will not be charged against the annual maximums under the new plan. Orthodontic coverage now has a lifetime benefit of $1500.

Cost Shares: Lower cost shares for E1 to E4 for endodontic, periodontic and oral surgery procedures. Cost sharing for other procedures has not changed.

New Procedures Covered: space maintainers; sealants on family members up through age 18; general anesthesia and IV sedation procedures; athletic mouth guards.

United Concordia suggests predetermination of benefits for the following non-emergency types of treatment: onlays, single crowns, prosthetics, periodontics, orthodontics, and oral surgery. This is to ensure that the patient has available coverage for these procedures so as to avoid an unexpected bill for these higher cost services.

It is very important to understand that the best way to optimize the value of these insurance benefits is to become "patients of record" with a local provider. This will greatly facilitate appointment and emergency care access.

Enrolled family members may obtain dental care from any dentist of their choice. Be reminded that a non-participating dentist may charge higher fees than the UCCI coverage allows. In this situation the patient will have to pay the cost difference, in addition to any cost share. Enrollees will get the greatest value from TFMDP coverage if they obtain care from a UCCI participating dentist.

If you need help locating a UCCI participating dentist, please stop by the Fort Detrick Dental Clinic for a list of Frederick area providers. You may also contact the CHAMPUS representative at 301-619-7175 or the Army Community Services at 301-619-2197 for assistance. Providers for a specific zip code can be located by accessing the UCCI web site at: www.ucci.com.
