Overseas Suitability Screening
Barquist Army Health Clinic provides overseas medical screenings for all active duty and family members. The medical exams are to verify medical readiness for the service member in preparation for the OCONUS PCS, current physical exams with preventative health screenings, immunizations for the projected assignment, and Exceptional Family Member Program(EFMP) Screening with verification of educational needs.
Service Members should receive the required medical forms from their Military Personnel Division. Service Members need a prescreening appointment with the EFMP office to determine all forms have been completed correctly for their screening and the type of screening appointment each family member needs for medical and EFMP clearance. Active duty Army Service Members are scheduled Prescreening appointments during their Levy Briefing.
All other service members may contact Andrea Lewis by email or phone to schedule the Prescreening appointment.
