What is the Patient-Centered Medical Home Model?
The Patient-Centered Medical Home Model (PCMH) is a primary-care model adopted throughout the Military Health System and in many civilian practices throughout the nation. It is healthcare as it should be - easy to access, patient-centered, team-based and quality-focused. This vision has begun at Barquist Army Health Clinic where we are committed to using evidence-based practices to provide the most effective treatment for our patient population.
You will be assigned to a specific primary care manager (PCM) that is on either Team Potomac or Team Shenandoah and you should see your PCM every time you visit the clinic. You will be provided with your PCM’s name when you enroll and each time you request an appointment. Each provider/nurse team has a designated phone line for you to use for non-emergency medical concerns. This continuity will provide a partnership for your healthcare.
Note: The Barquist Army Health Clinic will be Closed 3rd Friday of every month 1200-1630 for Training.
